INTRODUCTION: Borderline personality disorder is a commonly encountered disorder in psychiatry. This diagnosis is rarely given despite the benefits of its disclosure. Recommendations concerning diagnosis disclosure in psychiatry have recently been published. An analysis of the disclosure process in an outpatient team for young adults enables the identification of areas for improvement in this process, as well as nursing actions to facilitate health-illness transitions. METHOD: A clinical audit was conducted through a study involving a retrospective quantitative section based on an analysis of the computerized patient file and a qualitative section based on interviews with the team’s nurses. RESULTS: Of the 11 cases included, none met all 16 quality criteria. The patient’s abilities were taken into consideration in 100% of cases. The criteria concerning the presence of the patient’s family were rarely respected. The nurses’ own skills made it easier to support the patient during the diagnosis announcement. DISCUSSION: Conflicts between the patient and their family explain the difficulties encountered. The nursing actions carried out in this process are in line with those proposed by Meleis in her transitions theory. The actions proposed to improve practices will have to be evaluated.

RESUMO

Since September 2021, at the Paris Psychiatry and Neurosciences University Hospital Group, an advanced practice nurse has been providing post-emergency consultations for people who were initially seen in the emergency department, for whom an indication for outpatient care was given and who had problems accessing care. Collaboration with the nursing team is an important element not to be neglected in the implementation of this new profession.

RESUMO

Context : Nurses are regularly confronted with suicidal people. Their involvement is essential in the detection and prevention of suicide risk.Objectives : Through a literature review, we wanted to highlight the role of the advanced practice nurse by conducting specific training to improve nursing skills in the management of people with suicidal thoughts.Method : Our literature review was carried out according to the PRISMA recommendation criteria from several databases.Results : The review showed an improvement in confidence, attitudes, and nursing skills in assessing suicide risk. However, no long-term change in practice was demonstrated.Discussion : In France, a national training program on suicide risk prevention is implemented at the regional level by the Regional Health Agencies. At the same time, since 2019, Psychiatry and Mental Health has been part of the training program for advanced practice nurses.Conclusion : The advanced practice nurse is a central player in supporting teams in increasing their skills, particularly when conducting specific training.

The care of a patient experiencing a psychotic decompensation episode in an emergency department is complex. At the centre is a patient who is suffering and who requires care. This article presents the example of the treatment of a delirious and aggressive patient and its review of ways to improve practices.
